I'm unsure of the exact release date, but some sources list INSANITY with a 1996-02-25 release, while the CD was actually originally released on 1995-09-21-so, I'm going to assume the 1996 release date is actually this third press.

There are three presses of this album, and they can be hard to differentiate. This is the third press. The cover is a photo of the band, with a thick blue border, and the title “INSANITY” written in red text. The obi/title strip is opaque white. The text on the obi is the same as the second press (says “覇叉羅の恐るべきベールが 今、剥がされる。” along the side and “初回プレス完売!” at the top). The disc is white with purple artwork. Original copies featured a sticker on the shrink wrap reading “10,000枚突破” and “サードプレス” (I've only seen the sticker in pictures, and the text is too small to read completely). Copies with this sticker intact are impossible to find now.
